Notes to completing Application For A Student Award (FE1)

If you complete the form using a word processor, please keep within the constraints of the columns.

Forms returned by email or fax will not be accepted.

1. Indicate one of the three options and delete the others.
2. If divorced, complete details of absent parent.
3. Sign and date the bottom of the form.

4. Detail the student's Social Security Number as the application cannot be processed without it. If the student does not have a number, contact the Department of Employment & Social Security and they will oblige.

5. If you have been out of education for a number of years, detail what you have been doing since then, in particular the last 5 years.

6. Detail the proposed date of entry to the Institute.
7. If 'Yes' is stated to any of the questions relating to current or previous funding, give details and dates.
8. The student must sign and date the declaration.
